GHMC Birth Certificate for Unregistered Births
The following procedure has to be followed for Births which have not been registered. It is mostly applicable for cases where the birth has taken place many years back.
Procedure to be followed:
I)Walk into any eSeva center and fill in the application form available at e-seva for Non-availability GHMC Certificates of birth. Based on your details e-seva operator will verify the data whether the birth data is already registered?
II)If the record not found, applicant has to apply for non-availability certificates.
III)The following documents are required along with the filled application form.
Non-availability GHMC Certificates of Birth :
1. Ration card/any other residence proof.
2. School bonafied certificate.
3. Secondary school marks sheet.
4. Notarized affidavit on Rs 10/- Non-Judicial Stamped paper.
5. Any other documents if any.

Get your GMMC Birth certificates as soon as baby is born to avoid age proof issues.
If birth takes place at any hospital, such hospital authority has to inform the nearest GHMC ward office about the details of the birth in form No. I within 21 days of the birth to get ghmc birth certificate from eseva
If the birth takes place at home the head of the family has to inform the nearest GHMC ward office about the details of the birth in form No. I within 21 days of the birth
Later you should approach the ward office for GHMC birth certificate on any working day between 3 to 5 pm
You can get your GHMC birth certificate from the concerned ward office within three working daysIf there are any mistakes in your GHMC birth certificate like miss-pelt and other spelling mistakes, all the corrections are made on the request of parents and child only. To make corrections the following documents should be produced
| Declaration should be given by two gazetted officers ( The names of the gazetted officers are to be written in capitals) |
| An affidavit duly notarized on Rs. 10/- non judicial stamp paper. |
| Original birth certificate to be returned |
| Documentary evidence like Education Certificates, voter ID Card, Ration Card, Passport or any other registered documents. But the documents like PAN Card, LIC and Bank Statements are not accepted |
| Child’s name cannot be changed once registered |
